Anyone — a consumer, employee, competitor, regulator, or member of the public — may report a concern about a certified facility or operator, or about anyone falsely claiming to be Laser Safe Certified. Reports go directly to the Authority.

How to file

Email your report to the Authority.

Send your report to the address below. A member of the Authority reviews every report received.

Please include

  • The name of the facility or operator you are reporting, and its city and state.
  • Whether your concern is about a certified facility or operator, or about someone falsely claiming to be Laser Safe Certified.
  • What happened, including dates and any relevant detail.
  • Your name and a return email address, if you would like a response.

Please don’t include patient-identifiable medical information in your report.
